Phytocannabinoids—plant-derived cannabinoids, most notably cannabidiol (CBD) and delta-9-tetrahydrocannabinol (THC)—have been the subject of increasing scientific research for their potential to alleviate neuropathic pain, which includes conditions such as neuralgia and neuritis. Historically, cannabis extracts have been used for centuries in traditional medicine for pain relief, but modern clinical interest has focused on specific phytocannabinoids and their mechanisms of action. Preclinical studies in animal models have shown that both CBD and THC can modulate pain signaling through interactions with the endocannabinoid system, particularly CB1 and CB2 receptors, as well as through anti-inflammatory and neuroprotective effects.

Several randomized controlled trials (RCTs) and meta-analyses have evaluated the efficacy of cannabis-based medicines (including nabiximols, dronabinol, and medical cannabis) in neuropathic pain. These trials generally show modest but statistically significant improvements in pain scores compared to placebo. The National Academies of Sciences, Engineering, and Medicine (2017) concluded there is "substantial evidence" that cannabis is effective for chronic pain in adults, including neuropathic pain. However, studies are often limited by small sample sizes, short duration, heterogeneous formulations, and potential bias.

Overall, while there is a moderate level of scientific evidence supporting the use of phytocannabinoids in neuropathic pain conditions like neuralgia and neuritis, further high-quality, long-term studies are needed to determine optimal dosing, formulation, and long-term safety.
