The interest in Vitamin E for cognitive health stems from its antioxidant properties, as oxidative stress is believed to play a role in neurodegenerative diseases such as Alzheimer's disease. Some observational studies have found that individuals with higher dietary intake of Vitamin E have a lower risk of cognitive decline. Additionally, a few randomized controlled trials (RCTs) have explored the use of high-dose Vitamin E supplementation in individuals with mild to moderate Alzheimer's disease. For example, a well-known study published in 1997 (Sano et al., NEJM) found that high doses of alpha-tocopherol (2,000 IU/day) modestly delayed functional decline in Alzheimer's patients compared to placebo. However, results have been mixed, and subsequent studies have not consistently shown benefit, especially in cognitively healthy individuals or for prevention. The evidence for tocotrienols is even more limited, with most research focusing on animal models or in vitro studies rather than clinical outcomes. Overall, while there is some scientific rationale and limited clinical evidence for Vitamin E supporting brain function in specific populations, the overall quality and consistency of the evidence is low, and routine supplementation for memory or cognitive prevention is not broadly recommended.
