Specialized pro-resolving mediators (SPMs) are a group of bioactive lipid compounds derived from omega-3 and omega-6 fatty acids. They play a crucial role in actively resolving inflammation rather than simply suppressing it. Scientific interest in SPMs has grown over the past two decades, and preclinical studies have shown that SPMs—including resolvins, protectins, and maresins—can modulate immune responses and promote the resolution of lung inflammation. For example, animal and in vitro studies indicate that SPMs can reduce neutrophil infiltration, enhance macrophage-mediated clearance of debris, and limit tissue damage in models of acute lung injury, asthma, and pneumonia. Some early-phase clinical investigations have begun to evaluate SPMs’ effects in respiratory diseases, with preliminary results suggesting improved outcomes in inflammatory lung conditions. However, robust, large-scale human clinical trials are still lacking, and most evidence comes from laboratory and animal research. There is no longstanding traditional use of SPMs for lung health, as their discovery and characterization are relatively recent. In summary, there is a moderate scientific basis for the use of SPMs to support lung function, mainly grounded in mechanistic and animal studies, but clinical validation in humans remains limited.
