Cholic acid is a primary bile acid synthesized in the liver from cholesterol. Its medical use is specifically indicated for the treatment of certain rare genetic bile acid synthesis disorders, such as bile acid synthesis defects due to single enzyme deficiencies (e.g., sterol 27-hydroxylase deficiency, 3β-hydroxy-Δ5-C27-steroid oxidoreductase deficiency), and peroxisomal disorders such as Zellweger spectrum disorders. Scientific evidence supports its use in these settings because patients with these disorders cannot produce normal bile acids, leading to impaired bile flow, accumulation of toxic intermediates, and progressive liver disease. Supplementation with cholic acid helps restore the bile acid pool, improves bile flow, reduces liver damage, and supports normal growth and fat absorption in affected individuals. Multiple clinical studies and case reports have documented biochemical and clinical improvements, and cholic acid is FDA-approved for these indications. However, cholic acid is not indicated for general "liver detoxification" in healthy individuals or for non-genetic liver diseases. There is no robust evidence supporting its use for general detoxification or routine liver support outside of these rare, specific metabolic disorders. In summary, cholic acid is scientifically validated for a narrow range of inherited liver conditions, but not for general liver detoxification in the broader population.
