Terpinen-4-ol is a major active component of tea tree oil (Melaleuca alternifolia), which has been widely studied for its antimicrobial properties, particularly antifungal effects. Jock itch (tinea cruris) is a dermatophytic fungal infection, most commonly caused by Trichophyton species. Several in vitro studies have shown that terpinen-4-ol exhibits strong antifungal activity against dermatophytes, including those that commonly cause jock itch. For example, a study by Hammer et al. (2003) found that terpinen-4-ol was one of the most active constituents of tea tree oil against fungal pathogens responsible for skin infections.

Clinical studies on tea tree oil preparations (containing terpinen-4-ol as the main active ingredient) for various fungal skin infections, including athlete’s foot (tinea pedis), have demonstrated modest efficacy compared to placebo or standard treatments. While direct clinical trials evaluating terpinen-4-ol alone for tinea cruris are lacking, the antifungal mechanism—disruption of fungal cell membrane integrity—is well established in vitro. The evidence base is stronger for tea tree oil as a whole, but since terpinen-4-ol is its primary active constituent, some extrapolation is reasonable.

In summary, while there is no high-level clinical trial specifically for terpinen-4-ol in jock itch, its demonstrated antifungal activity and the supporting data from related dermatological conditions justify its consideration as a supportive treatment. However, it should not be considered a primary or sole therapy without further clinical evidence.
