Red Sage (Salvia miltiorrhiza, also known as Danshen) has a long history of use in Traditional Chinese Medicine (TCM), including for liver-related ailments. However, the evidence for its use in hepatitis is primarily based on limited scientific studies rather than just tradition. Several preclinical studies and a small number of clinical trials have investigated the effects of Red Sage and its active constituents (such as tanshinones and salvianolic acids) on liver injury and hepatitis models. These studies suggest that Red Sage may exert hepatoprotective, anti-inflammatory, and antioxidant effects, potentially reducing liver damage induced by toxins or viral infections.

A few randomized controlled trials and meta-analyses have examined Danshen-containing herbal formulas for chronic hepatitis B or liver fibrosis, reporting some improvements in liver function markers (such as ALT, AST) and histological findings. However, these studies often have methodological limitations, small sample sizes, and are frequently conducted alongside other herbal ingredients, making it difficult to attribute effects specifically to Red Sage.

Overall, while there is emerging scientific evidence that Red Sage may have beneficial effects on liver health and may support hepatitis treatment, the quality and quantity of clinical evidence is still limited. Larger, high-quality clinical trials are needed to confirm its efficacy and safety for hepatitis. Therefore, its use is best characterized as supported by preliminary scientific evidence and not solely traditional use.
