Phospholipids, particularly phosphatidylcholine, have scientific support for their use in supporting the hepatic (liver) system. Phosphatidylcholine is a major component of cell membranes, including hepatocytes (liver cells), and plays a crucial role in membrane integrity and repair. Several clinical and preclinical studies have explored the hepatoprotective effects of phospholipids, especially in conditions such as non-alcoholic fatty liver disease (NAFLD), alcoholic liver disease, and drug-induced liver injury. Research suggests that phospholipids can help restore membrane structure, enhance regeneration of hepatocytes, and reduce liver inflammation and fibrosis. For example, essential phospholipids (EPL) derived from soybeans have been used as adjunct therapy in liver diseases in Europe and Asia for decades, with some randomized controlled trials showing improved liver function tests and histology compared to placebo. However, results are mixed and not all studies confirm a significant clinical benefit, and meta-analyses highlight the need for more rigorous, larger trials. Despite this, the mechanistic rationale and the moderate body of clinical evidence justify the use of phospholipids for hepatic support, especially as a supportive therapy rather than a primary treatment. Regulatory agencies in some countries approve phospholipid preparations for liver health, further reflecting their established role.
