Pentacyclic triterpenoids are a diverse group of naturally occurring compounds found in many medicinal plants, including those traditionally used to support liver health, such as Phyllanthus niruri, Glycyrrhiza glabra (licorice), and Centella asiatica. Scientific research has indicated that certain pentacyclic triterpenoids, such as ursolic acid, oleanolic acid, and glycyrrhizin, exert hepatoprotective effects. These effects are generally attributed to their antioxidant, anti-inflammatory, and anti-fibrotic properties. For example, oleanolic acid and ursolic acid have demonstrated protective effects against chemically-induced liver injury in animal models by reducing oxidative stress and modulating inflammatory pathways (Liu J, 2013). Glycyrrhizin, a major triterpenoid saponin from licorice, is used in some countries as part of the treatment for chronic hepatitis and has been shown in clinical trials to reduce serum ALT levels and histological inflammation in hepatitis patients (van Rossum TG, 1999). While these findings are promising, the majority of evidence stems from preclinical studies, with fewer high-quality human clinical trials. The broad term "pentacyclic triterpenoids" encompasses many compounds with varying activities, so while some members of this group are scientifically validated for hepatic support, the evidence is not uniform across all pentacyclic triterpenoids. Thus, the evidence is moderate (rated 3), with specific compounds having demonstrated hepatoprotective effects supported by both animal and some human studies.
