Terminalia, particularly Terminalia arjuna, has a long history of use in traditional Ayurvedic medicine for supporting heart health and treating conditions described as "heart weakness." Ancient Ayurvedic texts such as the Charaka Samhita and Sushruta Samhita mention Arjuna bark as a "cardiac tonic," used for strengthening the heart muscle, managing palpitations, and alleviating symptoms associated with heart failure. The traditional use is often in the form of decoctions or powders of the bark.

There has been moderate scientific interest in Terminalia arjuna, with several small clinical studies and animal research suggesting potential cardioprotective effects. Some studies indicate improvements in cardiac function, reduction in angina frequency, and antioxidant activity. However, these studies are generally limited by small sample sizes, short durations, and methodological weaknesses. Comprehensive clinical validation is still lacking, and systematic reviews call for larger, long-term trials to establish efficacy and safety. Thus, while there is a strong traditional basis for the use of Terminalia arjuna in supporting heart health, scientific evidence remains preliminary and not robust enough for definitive clinical recommendations.
