There is emerging scientific interest in the use of vitamin D as a supportive treatment for cluster headaches, though the evidence is still limited and not fully established. Cluster headaches are severe, recurrent headaches that have been linked in some studies to alterations in circadian biology and possible deficiencies in certain micronutrients, including vitamin D. Some observational studies have reported lower levels of vitamin D in patients with cluster headaches compared to healthy controls. It is hypothesized that vitamin D, which plays roles in immune modulation and neuroprotection, may help regulate the hypothalamus—the brain region implicated in cluster headache pathophysiology.

A few small clinical case series and open-label studies have suggested that vitamin D supplementation may reduce the frequency or severity of cluster headache attacks, especially in individuals who are deficient. For instance, one case series published in 2014 reported some benefit with high-dose vitamin D in cluster headache patients, though these studies are limited by small sample sizes and lack of placebo control. At present, there are no large, well-controlled randomized trials confirming efficacy, and major headache societies have not formally recommended vitamin D as a primary therapy. Nevertheless, due to its relatively good safety profile and the commonality of vitamin D deficiency, supplementation may be considered in deficient individuals, but should not replace established treatments.
