Sage oil (Salvia officinalis) has a long history of use in traditional medicine systems, particularly in Europe, for supporting the female reproductive system. Folk remedies have used sage to help address menstrual irregularities, alleviate menstrual cramps, and manage symptoms associated with menopause such as hot flashes and excessive sweating. The traditional rationale is partly due to its phytoestrogenic compounds, which are plant-derived substances that may mimic some effects of estrogen in the body. Sage has also been used as an emmenagogue, believed to stimulate menstrual flow, and to help reduce excessive lactation (anti-galactagogue).

Despite these traditional uses, scientific evidence supporting sage oil's efficacy for female reproductive health is limited and largely preliminary. Most studies focus on sage extracts rather than the essential oil itself. Some small clinical trials and laboratory studies suggest potential benefits of sage (particularly as a tea or extract) in reducing menopausal symptoms, but these findings are not robust or widely replicated. There is little to no quality research specifically on sage essential oil administered aromatically or topically for these purposes. Safety concerns also exist, as sage oil contains thujone, a compound that can be neurotoxic in high doses. Due to a lack of rigorous clinical trials, the use of sage oil for the female reproductive system remains primarily supported by traditional rather than scientific evidence.
