Vitamin B5 (pantethine) is a derivative of pantothenic acid that serves as a precursor to coenzyme A, an essential molecule in fatty acid metabolism. There is scientific rationale for its use in supporting fat metabolism due to its biochemical role in the β-oxidation of fatty acids. Several clinical studies have evaluated pantethine supplementation for its effects on lipid profiles. Notably, some trials have demonstrated that pantethine can modestly lower total and LDL cholesterol and triglycerides, while potentially raising HDL cholesterol. For example, a randomized, placebo-controlled study published in Nutrition Research (2011) found that pantethine supplementation led to significant improvements in lipid parameters in subjects with low to moderate cardiovascular risk.

However, while these studies suggest pantethine may favorably modulate lipid metabolism, direct evidence for its use in treating "poor" fat metabolism as a clinical disorder (such as inborn errors of metabolism or metabolic syndrome) is limited. Most studies focus on dyslipidemia rather than direct measures of fatty acid oxidation or metabolic rate. There is a lack of large-scale, long-term trials definitively proving its efficacy for improving fat metabolism in otherwise healthy individuals. Therefore, while the mechanistic basis is strong and some evidence exists for lipid-lowering effects, the overall strength of clinical evidence supporting its use specifically for poor fat metabolism is moderate at best.
