4-Hydroxyandrostenedione (also known as formestane) is a steroidal aromatase inhibitor that has been used clinically to support the estrogen body system, primarily in the context of hormone-dependent breast cancer. Aromatase is the enzyme responsible for the conversion of androgens to estrogens. By inhibiting aromatase, 4-hydroxyandrostenedione significantly reduces estrogen production in the body. This property led to its approval and use as a treatment for postmenopausal women with estrogen receptor-positive breast cancer. Numerous randomized clinical trials and meta-analyses have demonstrated its efficacy in lowering systemic estrogen levels and reducing tumor progression in this context. While newer, non-steroidal aromatase inhibitors (like anastrozole and letrozole) have largely replaced formestane due to improved oral bioavailability, the mechanism and clinical effects of 4-hydroxyandrostenedione are well documented in the scientific literature.
