Trichosanthes, particularly Trichosanthes kirilowii (commonly known as "gua lou" or "Chinese snake gourd"), has a long history of use in Traditional Chinese Medicine (TCM). In TCM, it is believed to moisten the lungs, clear phlegm, and relieve symptoms such as cough and shortness of breath, which are also present in emphysema and other chronic lung conditions. Emphysema itself is not described in classical Chinese texts, but the symptom patterns (such as persistent cough, difficulty breathing, and phlegm production) are recognized and treated with herbs like Trichosanthes. Modern pharmacological research on Trichosanthes focuses primarily on its anti-inflammatory, expectorant, and bronchodilatory effects, but direct clinical studies on its efficacy for emphysema are limited. Most of the evidence remains preclinical or anecdotal, with some small studies suggesting that compounds isolated from the fruit or root may help reduce airway inflammation and facilitate mucus clearance. However, these findings are not robustly validated in large, high-quality clinical trials specifically involving emphysema patients. In summary, the use of Trichosanthes for emphysema is justified mainly by tradition, with some preliminary scientific rationale but insufficient clinical evidence. Its inclusion in herbal formulas for chronic lung diseases in TCM remains common practice, but patients should be aware that scientific validation is still lacking.
