Vitamin D (ergocalciferol, or vitamin D2) has been investigated for its potential role in supporting cognitive health and treating dementia, but the scientific evidence is limited and mixed. Several observational studies have found associations between low serum vitamin D levels and increased risk of cognitive decline or dementia, including Alzheimer's disease. The proposed mechanisms include vitamin D’s role in neuroprotection, reducing neuroinflammation, and supporting neuroplasticity. However, these associations do not establish causation, and it is unclear whether vitamin D deficiency is a contributing factor to dementia or simply a marker of poor health among the elderly.

Randomized controlled trials (RCTs) addressing the effectiveness of vitamin D supplementation (including ergocalciferol) for preventing or treating dementia have yielded inconsistent results. Some small studies suggest marginal cognitive benefits, while larger or more rigorous trials generally fail to show significant improvement in cognitive outcomes. Current clinical guidelines do not recommend vitamin D supplementation specifically for dementia treatment, though correcting deficiency is advised for overall health. The evidence is stronger for vitamin D3 (cholecalciferol) than for D2 (ergocalciferol), but even for D3, definitive benefit in dementia is unproven.

In summary, while there is some scientific interest and limited evidence linking vitamin D status and cognition, the use of ergocalciferol specifically to treat dementia is not well-supported by high-quality clinical trials. Its use for this condition should be considered experimental and secondary to correcting deficiency for general health.
