Huperzine-A is an alkaloid extracted from the Chinese club moss Huperzia serrata. Its use for dementia, particularly Alzheimer's disease, is primarily supported by scientific interest rather than longstanding tradition. Huperzine-A acts as a reversible acetylcholinesterase inhibitor, increasing the availability of acetylcholine, a neurotransmitter important for memory and learning. This mechanism is similar to approved Alzheimer's drugs like donepezil.

Several randomized, controlled trials—mostly conducted in China—have found that Huperzine-A can improve cognitive function and daily living activities in patients with Alzheimer's disease and other forms of dementia. Meta-analyses and systematic reviews (e.g., Cochrane, 2008) suggest modest benefits over placebo in terms of cognitive scores and global clinical assessment, though the methodological quality of many studies is low, with small sample sizes and short durations. Safety appears acceptable in the short-term, but long-term data are lacking.

While Huperzine-A is not approved for dementia treatment in most countries, it is available as a dietary supplement and is sometimes used as an alternative therapy. Major medical guidelines do not endorse its routine use due to inconsistent evidence and the need for larger, high-quality trials. Therefore, while there is a scientific rationale and some clinical evidence, the overall quality and quantity of research limit its recommendation for dementia at this time.
