Agnuside is one of the main active iridoid glycosides found in the fruit of Vitex agnus-castus (chaste tree). The use of Vitex agnus-castus for menstrual disorders, including cramps (dysmenorrhea), has a long history in traditional medicine. Scientifically, studies have primarily focused on the whole extract rather than isolated agnuside. Several randomized controlled trials and meta-analyses indicate that Vitex agnus-castus extract can reduce symptoms of premenstrual syndrome (PMS), such as breast pain, irritability, and mood changes, and some studies also suggest benefit for menstrual cycle regulation. However, direct evidence specifically linking agnuside alone to the reduction of menstrual cramps is limited.

The proposed mechanism involves modulation of prolactin levels through dopaminergic activity, which may indirectly reduce symptoms associated with PMS and potentially menstrual cramps. Most clinical studies use standardized extracts containing agnuside (often quantified at 0.5% by weight), but do not separate out its effects from the other active constituents. Thus, while there is scientific evidence for Vitex agnus-castus extracts in the treatment of menstrual discomfort, the evidence supporting agnuside alone for the treatment of menstrual cramps is weak, and most conclusions are extrapolated from studies of the whole plant extract.

In summary, agnuside is a marker and likely one of several active compounds in Vitex agnus-castus extracts used to treat menstrual cramps, but clinical evidence for agnuside itself is limited and indirect.
