Sage-leaf alangium (Alangium salviifolium) has a history of use in traditional medicine systems, particularly in Ayurveda and Unani medicine, for the treatment of respiratory ailments, including coughs. The concept of "damp" cough does not have a direct equivalent in these systems, but the plant is often used for phlegmatic or productive coughs, which may correspond to "damp" in traditional Chinese medicine. Various parts of the plant, such as the bark and roots, have been used in decoctions and infusions to help clear mucus, relieve chest congestion, and ease coughing. The traditional use is documented in classical Ayurvedic texts and some ethnobotanical surveys.

However, scientific evidence supporting its efficacy for cough is limited. Some preliminary phytochemical studies have identified the presence of alkaloids, flavonoids, and saponins, which may have anti-inflammatory or expectorant properties, but clinical trials or robust pharmacological studies specifically targeting cough relief are lacking.
