Ornithine alpha-ketoglutarate (OKG) has been researched as a nutritional supplement to support recovery during convalescence, particularly following surgery, trauma, burns, or severe illness. The rationale for its use is based on the roles of its constituents: ornithine, an amino acid involved in the urea cycle and in promoting anabolic processes, and alpha-ketoglutarate, a key intermediate in the Krebs cycle and protein metabolism. Together, OKG is thought to enhance protein synthesis, reduce protein catabolism, and support wound healing.

Several clinical studies, mostly in the 1980s and 1990s, have investigated OKG supplementation in hospitalized patients or those recovering from major illness. Some trials reported that OKG supplementation led to improved nitrogen balance, greater lean body mass retention, and shorter hospital stays, particularly in elderly or malnourished patients. A 2001 review in Nutrition acknowledged potential benefits but noted that the evidence was limited by small sample sizes and methodological inconsistencies.

Overall, while there is some scientific evidence supporting the use of OKG in convalescence, it is not robust. The quality and quantity of studies are limited, and more recent, large-scale, placebo-controlled trials are lacking. Thus, its use is sometimes included in clinical nutrition guidelines in Europe, but it is not universally accepted or recommended. In summary, OKG’s use for supporting convalescence is based on plausible mechanisms and some supportive clinical evidence, but its efficacy is not definitively proven.
