N-acetyl-cysteine (NAC) has some scientific evidence supporting its use in the context of colitis, but this evidence is limited and primarily preclinical. NAC is a precursor to glutathione, a major intracellular antioxidant, and has been investigated for its ability to reduce oxidative stress and inflammation—key mechanisms in the pathogenesis of colitis, including both ulcerative colitis and Crohn’s disease. Several animal studies have demonstrated that NAC administration can decrease colonic inflammation, reduce oxidative damage, and improve histological outcomes in models of chemically-induced colitis (such as dextran sulfate sodium-induced colitis in mice). These beneficial effects are attributed to NAC’s antioxidant properties and its ability to modulate inflammatory cytokine production. There are a few small-scale human studies and case reports suggesting that NAC may be helpful as an adjunct therapy in inflammatory bowel disease (IBD), but robust, large-scale clinical trials are lacking. A handful of pilot trials have examined oral NAC supplementation in patients with ulcerative colitis, showing some improvement in clinical and biochemical markers, but results are inconsistent and not definitive. Overall, while NAC is not a standard treatment for colitis, its mechanistic rationale and preliminary evidence suggest potential benefit. However, current evidence is insufficient for routine clinical use, and further research is needed to establish efficacy and safety.
