Phospholipids, particularly in the form of essential phospholipids (EPLs) such as phosphatidylcholine, have been used as supportive therapy in chronic liver diseases, including cirrhosis. The rationale is based on the role of phospholipids as fundamental components of cell membranes, including hepatocytes. Some studies, primarily from Eastern Europe and Asia, have investigated the use of EPLs in chronic liver conditions. A few randomized controlled trials and meta-analyses suggest that phospholipid supplementation may improve certain biochemical markers of liver function (such as transaminase levels) and histological features, potentially by stabilizing cell membranes, promoting regeneration, and exerting anti-fibrotic effects. However, the quality and consistency of evidence are limited.

Major international guidelines (such as EASL or AASLD) do not currently recommend phospholipids as standard therapy for cirrhosis, largely due to insufficient robust clinical evidence for meaningful clinical outcomes like survival or reversal of cirrhosis. Most available studies are small, sometimes methodologically weak, and often industry-sponsored. Thus, while there is a scientific rationale and some clinical investigation, the evidence base is modest at best, justifying a rating of 2 out of 5. The use of phospholipids for cirrhosis remains an adjunct or supportive measure rather than a primary or well-validated treatment.
