Vitamin E, particularly in its "mixed" form (containing various tocopherols and tocotrienols), has been investigated for its potential role in supporting or treating cardiovascular disease (CVD). The scientific rationale is based on Vitamin E's antioxidant properties, which could theoretically protect lipids from oxidative damage, a key process in the development of atherosclerosis. Early observational studies suggested that higher Vitamin E intake was associated with lower rates of heart disease, leading to widespread interest in supplementation. However, subsequent large randomized controlled trials (RCTs), such as the Heart Outcomes Prevention Evaluation (HOPE) and the Women's Health Study, did not find significant reductions in cardiovascular events with Vitamin E supplementation in at-risk populations. Some studies even suggested potential harm, such as increased risk of heart failure or hemorrhagic stroke with high-dose supplementation. Meta-analyses of RCTs have largely concluded that Vitamin E does not reduce the risk of major cardiovascular events. Despite these findings, Vitamin E continues to be marketed for "heart health," though leading health authorities do not recommend its use for CVD prevention or treatment. In summary, while there is a scientific basis for antioxidant therapy in CVD, the clinical evidence supporting Vitamin E supplementation for this purpose is weak, hence the evidence rating of 2 out of 5.
