Mannose is a type of simple sugar (monosaccharide) that has gained attention for its role in supporting bladder health, particularly in the prevention and management of urinary tract infections (UTIs). The primary scientific basis for its use comes from clinical studies that demonstrate mannose's ability to inhibit the adhesion of Escherichia coli (E. coli) bacteria—the most common cause of UTIs—to the urothelial cells lining the bladder. E. coli bacteria adhere to these cells via structures known as fimbriae, which recognize and bind to mannose residues present on the bladder wall. Supplementing with D-mannose saturates the urinary tract with free mannose, which the bacteria bind to instead, allowing them to be flushed out during urination rather than attaching to the bladder lining.

Multiple randomized controlled trials and systematic reviews have evaluated the efficacy of D-mannose for preventing recurrent UTIs, particularly in women. Evidence suggests that D-mannose supplementation may reduce the recurrence rate of UTIs, with a safety profile superior to that of long-term antibiotic use. However, while promising, the body of evidence is still developing and some studies are limited by small sample sizes or lack of blinding. Overall, D-mannose is one of the few natural ingredients with moderate scientific support for its use in urinary tract and bladder health, meriting a mid-level evidence rating.
