Vitamin B3, specifically in the form of nicotinamide, has garnered scientific interest as a potential therapeutic agent for Alzheimer’s Disease (AD), but the evidence for its effectiveness remains limited and preliminary. The rationale for its use is based on laboratory and animal studies showing that nicotinamide can influence cellular processes associated with AD pathology. Notably, a 2008 study published in the Journal of Neuroscience demonstrated that nicotinamide administration improved cognitive function and reduced tau pathology in a mouse model of Alzheimer’s Disease. Researchers hypothesize that nicotinamide’s ability to inhibit certain enzymes (such as sirtuins and poly(ADP-ribose) polymerases) and its role in cellular energy metabolism may help protect neurons from degeneration and support synaptic function. However, human clinical trials are scant. A small pilot study (Safety of Nicotinamide in Alzheimer Disease, ClinicalTrials.gov NCT00580931) found that high doses were generally well-tolerated, but evidence for clinical benefit was not robust or conclusive. No large-scale, phase III trials have confirmed a therapeutic benefit of nicotinamide in AD patients. Current Alzheimer’s treatment guidelines do not recommend nicotinamide as a standard therapy outside of research settings. In summary, while there is a scientific rationale and preclinical evidence supporting the investigation of nicotinamide for Alzheimer’s Disease, the clinical evidence in humans is insufficient to recommend its use for this condition at present.
