Vincamine is an indole alkaloid derived from the plant Vinca minor (lesser periwinkle) and has been used primarily in Europe as a cerebral vasodilator, intended to enhance blood flow to the brain. Its main documented pharmacological actions are vasodilation and mild improvement of cerebral metabolism. Vincamine has also been shown in some in vitro and animal studies to modulate acetylcholine release and turnover, likely by affecting neuronal metabolism and possibly influencing cholinergic neurotransmission. However, the evidence for these effects in humans is limited and mostly indirect.

Clinical studies from the 1970s and 1980s investigated vincamine for cognitive impairment and dementia, conditions often associated with cholinergic dysfunction. While some trials reported modest improvements in cognitive function, these effects were not consistently linked to direct enhancement of acetylcholine levels, but rather to improved cerebral perfusion. There is insufficient robust clinical evidence showing vincamine directly supports or enhances the acetylcholine system in humans. As a result, while there is some scientific rationale and preclinical evidence for its influence on acetylcholine pathways, the overall clinical evidence remains weak. Vincamine’s use for supporting the acetylcholine system is therefore supported by limited scientific data, justifying a low evidence rating.
