Cluster headaches are a rare but extremely painful type of headache that occur in cyclical patterns or clusters, typically lasting for weeks or months, followed by periods of remission. They are considered one of the most severe forms of primary headache and are often described as a piercing, burning, or drilling pain, usually focused around or behind one eye.

Unlike migraines, cluster headaches are:

  • Unilateral (one-sided)

  • Shorter in duration (15 minutes to 3 hours)

  • More frequent (up to 8 times a day during a cluster period)

  • Accompanied by autonomic symptoms such as:

    • Tearing or redness of the eye

    • Nasal congestion or runny nose on the affected side

    • Eyelid drooping or swelling

    • Restlessness or agitation

The exact cause is not fully understood, but cluster headaches are believed to involve abnormal activity in the hypothalamus, affecting circadian rhythms and trigeminal nerve pathways. Triggers may include alcohol, strong odors, heat, stress, and changes in sleep patterns.

If you experience recurrent, severe, one-sided headaches with eye symptoms or restlessness, seek evaluation by a neurologist. Cluster headaches are often misdiagnosed as migraines or sinus headaches and require specific treatment.
