Vitamin D has been investigated as a potential supportive agent in the management of hypertension due to its role in calcium metabolism and vascular health. Epidemiological studies have shown that low serum levels of vitamin D are associated with increased risk of hypertension and cardiovascular disease. Mechanistically, vitamin D is thought to modulate the renin-angiotensin-aldosterone system (RAAS), which plays a central role in blood pressure regulation. Vitamin D deficiency may lead to increased renin secretion, resulting in higher blood pressure.
However, interventional studies and randomized controlled trials (RCTs) have produced mixed results. Some smaller studies and meta-analyses suggest a modest blood pressure-lowering effect of vitamin D supplementation, particularly in individuals who are vitamin D deficient or have pre-existing hypertension. For example, a 2019 meta-analysis in the Journal of Human Hypertension reported a slight reduction in systolic blood pressure with vitamin D supplementation, though the effect was small and not consistently observed across all studies.
Overall, the evidence does not robustly support vitamin D supplementation as an effective standalone treatment for hypertension in the general population. Major guidelines do not recommend vitamin D solely for blood pressure control, but it may be considered in individuals with confirmed deficiency. The evidence base is rated as weak to moderate (2/5), largely due to inconsistent findings and small effect sizes.
