Vitex Agnus-Castus (Chasteberry): A Comprehensive Reference

1. Identity and Botanical Classification

Scientific name: Vitex agnus-castus L. Family: Lamiaceae (formerly classified under Verbenaceae). Common names: Chaste tree, chasteberry, Abraham's balm, lilac chastetree, and monk's pepper. In German-language literature and on product labels it is frequently rendered as Agni casti fructus (the dried ripe fruit), the form named in European pharmacopoeial documents.

The plant is a deciduous shrub or small tree native to the Mediterranean region, western Asia, and parts of Africa. It typically grows 8 to 20 feet (2.4 to 6 meters) tall with an upright, rounded form, featuring aromatic, palmately compound leaves composed of five to seven leaflets that are grayish-green above and gray beneath, often fragrant when bruised. The plant produces spikes of small, fragrant lavender to violet flowers in summer, followed by small, pepper-like berries that are the primary medicinal part.

Modern plant taxonomy recognizes that the genus Vitex comprises approximately 223 species. It is one of the few temperate-zone species of Vitex, which is on the whole a genus of tropical and subtropical flowering plants. At present, V. agnus-castus grows on the East, South, and West coasts of the United States, and is also found in northeastern India, Afghanistan, Pakistan, and Iran.

Part Used and Principal Commercial Forms

The ripe, dried fruit (berry) is the part almost exclusively used medicinally. Vitex agnus-castus (VAC), commonly known as chaste tree, is one of the most widely utilized botanical agents for women's health. Commercial preparations include:

  • Standardized dry extracts in tablet or capsule form — the form used in the majority of clinical trials (e.g., extract Ze 440 and BNO 1095).
  • Oral drops / liquid extract — used in several European licensed products (e.g., Mastodynon® and Cyclodynon®).
  • Crude dried fruit powder in capsules.
  • Tinctures prepared in ethanol.
  • Tea infusions from crushed dried berries, representing a lower-potency traditional form.

Standard reference constituents for standardization include the iridoid glycoside agnuside and the flavonoid isovitexin; other flavonoids (casticin, orientin), iridoid glycosides (aucubin, eurostoside), and terpenoids (cineole, sabinene, limonene, camphene) are also present.


2. Traditional and Historical Use

Vitex agnus-castus has a rich history of use in traditional medicine across various cultures for over 2,500 years, particularly for gynecological issues. The historical use of chasteberry has been dated back over 2,000 years to the ancient Greeks, Romans, and Egyptians.

Ancient Greece and Rome

Theophrastus mentioned the shrub several times, as ágnos (ἄγνος) in his Enquiry into Plants. In ancient Greece, the tree was called agnos, meaning holy, pure, and chaste. They dedicated it to their mother goddess Demeter. At the festival of Thesmophoria held in her honour, Greek women, who aimed to remain chaste for the occasion, strewed their beds with branches of the tree, and wore garlands of it in the day, so that the aroma might subdue the ardour of any would-be suitor. The ancient Romans dedicated the vitex tree to the goddess Hera, the protector of motherhood and marriage, who was said to have been born under one. Roman wives were also said to use the leaves to reduce the libido of their husbands. Vestal virgins carried twigs of the tree as a symbol of chastity.

It has long been believed to be an anaphrodisiac — leading to its name as "chaste tree" — but its effectiveness for such action remains unproven.

Medieval Christian Europe

During the medieval period in Christian Europe, the plant earned the moniker "monk's pepper" due to its use by monastic communities, where dried berries were consumed to support vows of celibacy by curbing lustful impulses. This tradition tied into broader folklore where Vitex agnus-castus symbolized purity and served as a protective talisman against temptation, often referenced in herbals as a spiritual aid for moral discipline.

Gynecological and Lactation Uses

Records note that chasteberry was used in many cultures to support gynecological health, skin health, digestive comfort, and overall well-being. Vitex was also believed to be a supportive tool for lactation in nursing mothers. Historically, preparations of the fruit were described as useful for regulating menstruation, expelling the placenta, and stimulating milk production — uses that parallel, at least in part, its modern scientific investigation.

The fruit has been used for more than 2,500 years as a phytotherapeutic agent. In the last century, VAC has been mostly used for the treatment of premenstrual syndrome (PMS), menstrual irregularities, fertility disorders, and symptoms of menopause.


3. Key Constituents and Active Compounds

VAC is rich in diverse bioactive constituents, including flavonoids, iridoids, diterpenes, and essential oils, which collectively contribute to its complex pharmacological profile. According to current data, the presence of 20 flavonoids, 10 iridoids, 11 phenolic acids, three diterpene alkaloids, and 12 terpenes has been confirmed in the extract of Vitex fruits. Scientists have discovered over 200 active plant compounds in Vitex, including flavonoids, terpenoids, steroids, iridoids, and lignans.

Flavonoids

The flavonoid fraction includes casticin (a polymethoxylated flavone), vitexin, isovitexin, orientin, and penduletin. Vitex extract component analysis has confirmed the presence of vitexin, rotundifuran, kasticin (casticin), artemetin, and some phytoestrogens. The flavonoid apigenin has been identified as the most active ERβ-specific phytoestrogen in VAC. Other isolated compounds include vitexin and penduletin, and the outcome shows that the phytoestrogens in VAC are ERβ-specific.

Iridoid Glycosides

Iridoids, particularly agnuside and aucubin, are present and are used as reference markers for standardization. Some standardized extracts are calibrated to a specified content of aucubin (e.g., 0.42–0.82 mg per dosage unit), used as a quantitative marker.

Diterpenes and Triterpenes

The search for the chemical identity of the dopaminergic compounds resulted in isolation of a number of diterpenes, of which some clerodadienols were most important for the prolactin-suppressive effects. Viteagnusin I emerged as the most potent diterpene (EC₅₀: 6.6 µM), followed by rotundifuran (EC₅₀: 12.8 µM), whereas vitexilactone was inactive. Triterpenes were also identified as active, with 3-epi-maslinic acid being the most active compound (EC₅₀: 5.1 µM).

Essential Oil Constituents

Secondary metabolites include terpenoids (the main components of the essential oil), flavonoids, iridoids, and phenol carboxylic acids. Common monoterpenes in the essential oil include 1,8-cineole, sabinene, limonene, and camphene.


4. Mechanisms of Action

Vitex agnus-castus and its constituents affect a number of physiological actions via multiple extra- and intracellular mechanisms of action. The principal pharmacologically relevant mechanisms are described below.

Dopaminergic Activity and Prolactin Inhibition

This is the most thoroughly characterized mechanism. Pre-clinical studies suggest that extracts prepared from the fruits of VAC interact with dopamine D2 receptors, leading to reduced prolactin secretion. Dopaminergic activity was evaluated using radioligand binding assays and via the inhibition of prolactin release from rat pituitary cells. Dopamine (DA) plays a crucial role in regulating the synthesis and secretion of prolactin (PRL). DA is a potent inhibitor of PRL secretion (prolactin inhibiting factor; PIF), which is achieved through direct interactions with D2 receptors present on the membranes of lactotrophic cells. It has been postulated that the diterpenes contained in VAC extract may interact with dopamine D2 receptors (D2R) and inhibit prolactin release via dopamine D2R activation in the anterior pituitary.

Research demonstrated for the first time that AC extract contains an active principle that binds to the D2 receptor, and it is very likely that it is this dopaminergic principle which inhibits prolactin release in vitro from rat pituitary cells. The dopaminergic activity of VAC diterpenes appears to depend on the configuration and on ring substitution in the side chain. This also highlights for the first time the dopaminergic contribution of triterpenes such as 3-epi-maslinic acid.

Casticin and the diterpenes are believed to reduce elevated prolactin levels. Radioligand studies of Vitex extracts confirm that diterpenes bind to type 2 dopamine receptors and to all subtypes of opiate receptors. The diterpene clerodadienol binds dopamine receptors and suppresses prolactin release as effectively as dopamine. Because dopamine suppresses pituitary secretion of both LH and prolactin, Vitex also reduces LH release and thereby reduces serum testosterone levels, which can be therapeutic in women with polycystic ovary syndrome.

Opioidergic Activity

VAC has been shown to act as an agonist at the mu-opiate receptor. This mu-opioid receptor activity is thought to contribute to the herb's effects on the central regulation of luteinizing hormone (LH) and other aspects of the hypothalamic-pituitary-gonadal axis.

Phytoestrogenic Activity

Linoleic acid present in the fruits possesses the ability to bind to estrogen receptors and cause some estrogen gene induction. Estrogenic activities are mediated by means of estrogen receptors (ER). The flavonoid apigenin was identified as the most active ERβ-specific phytoestrogen in VAC. There is no evidence to support that A. castus has anti-estrogenic activity, and in fact it may have weak estrogenic activity.

Additional Pharmacological Properties

Extracts and their components are characterized by anti-inflammatory, antiproliferative, antioxidant, antimicrobial (particularly antifungal), vasoprotective, and analgesic effects. Emerging evidence from in vitro, in vivo, and clinical studies suggests that VAC may modulate key neuroendocrine pathways involved in menopause through dopaminergic, phytoestrogenic, opioidergic, and indirect serotonergic mechanisms.


5. Scientific Evidence by Area of Use

5.1 Premenstrual Syndrome (PMS)

PMS is by far the indication with the largest clinical evidence base for VAC, and the area in which regulatory bodies have granted formal approval.

Regulatory standing: The German health authorities have approved the use of V. agnus-castus for menstrual cycle irregularities, premenstrual syndrome, mastalgia, and menopausal disorders. The EU herbal monograph (EMA/HMPC) designates the product a "herbal medicinal product for the treatment of premenstrual syndrome" under well-established use.

Key randomized controlled trial: A landmark randomized, placebo-controlled study by Schellenberg (2001), published in BMJ, found VAC extract significantly superior to placebo across three menstrual cycles for a broad range of PMS symptoms. Systematic review (Van Die et al., 2013): A systematic review evaluated evidence for efficacy and safety of Vitex extracts from randomized controlled trials. Thirteen randomized controlled trials were identified and twelve included in the review, of which eight investigated PMS, two premenstrual dysphoric disorder, and two latent hyperprolactinaemia.

Meta-analysis (Csupor et al., 2019): Although chasteberry has been studied in several clinical trials, the efficacy of properly characterized preparations had not previously been assessed in meta-analyses. Out of 21 clinical trials, three studies (520 females) fulfilled the inclusion criteria, comparing special extracts Ze 440 and BNO 1095 to placebo for the treatment of PMS. VAC preparations were confirmed to be effective in the reduction of PMS symptoms: women taking VAC were 2.57 (95% CI 1.52–4.35) times more likely to experience a remission in their symptoms compared to those taking placebo. However, the majority of clinical trials cannot be used as evidence for efficacy due to incomplete reporting, especially concerning the description of the medication used. More trials following the CONSORT recommendations are needed to assess the efficacy of VAC extracts.

Meta-analysis (AJOG, 2017): A systematic review and meta-analysis found 17 randomized controlled trials of Vitex agnus castus in the treatment of PMS, of which 14 could be included in the quantitative analysis.

Overall evidence strength: Some research has suggested that chasteberry might reduce PMS symptoms such as breast pain or tenderness, but higher-quality evidence is needed to reach any definitive conclusions, per the U.S. National Center for Complementary and Integrative Health (NCCIH). The European regulatory position (EMA) is more permissive, granting well-established use status for PMS.

5.2 Premenstrual Dysphoric Disorder (PMDD)

In a study, patients with PMDD were treated with fluoxetine and Vitex agnus-castus. Fluoxetine helped to treat psychological symptoms while the VAC extract reduced the physical symptoms. The comparative trial by Atmaca et al. (2003) in Human Psychopharmacology (fluoxetine versus Vitex in PMDD) is cited as an important reference in this area. Evidence for PMDD remains more limited than for PMS and has been based on relatively small trials.

5.3 Cyclic Mastalgia (Breast Pain)

The German Commission E has approved the use of A. castus for mastalgia. The evidence of effectiveness of A. castus in the management of mastalgia is available from both randomized and non-randomized studies.

A systematic review and meta-analysis concluded that VAC is a safe and effective treatment option for cyclic mastalgia, but that more high-quality clinical trials are needed to strengthen the evidence base. One double-blind, placebo-controlled clinical study in patients with premenstrual mastodynia examined serum prolactin levels; prolactin levels were indeed reduced in the patients treated with the extract.

Real-world cohort data: A retrospective study analyzed data from 1,700 women with a mean age of 30.2 years. The most common menstrual cycle disorders were dysmenorrhea (43.8%) and mastodynia/mastalgia (21.1%). Three-month treatment with VAC extract substantially decreased the percentage of patients with irregular cycles (from 9.1% to 0.1%) and breast tenderness (from 39.9% to 0.8%).

5.4 Latent Hyperprolactinaemia

The activity of VAC extract constituents could be localized within the pituitary–gonadal axis. Research in pituitary cell assays elucidated a dopaminergic inhibition of prolactin synthesis and/or release. The effective administration of ethanol seed extracts against mastodynia and symptoms related to female cycle disorders with concomitant hyperprolactinaemia has been documented. The systematic review by Van Die et al. (2013) included two randomized controlled trials specifically on latent hyperprolactinaemia. Some degree of hyperprolactinaemia may be observed in patients with PMS, menstrual irregularities, and fertility disorders, and VAC effects on hyperprolactinaemia have been assessed in a small number of studies and in some patient series or single case reports.

Evidence strength: The dopaminergic mechanism is biologically plausible and supported by pre-clinical data, and there are clinical studies showing prolactin reduction; however, the evidence base from well-controlled trials remains limited in size.

5.5 Menopause Symptoms

A preliminary study suggested that chasteberry might help with some menopausal symptoms, but more research is needed to study this potential effect (NCCIH, NIH). One randomized controlled double-blind clinical trial aimed to determine the effectiveness of Vitex agnus-castus extracts in alleviating menopausal symptoms. The study group comprised 52 women; participants were randomly divided into a Vitex group (26) and a placebo group (26). Menopausal symptoms were assessed using the Greene Scale before and 8 weeks after the intervention.

Emerging evidence from in vitro, in vivo, and clinical studies suggests VAC may modulate key neuroendocrine pathways involved in menopause through dopaminergic, phytoestrogenic, opioidergic, and indirect serotonergic mechanisms. The overall evidence base for menopause is preliminary and trial sizes have been small.

5.6 Polycystic Ovary Syndrome (PCOS)

A randomized controlled trial investigated the efficacy of standardized VAC extract in managing features of PCOS. The primary aim was to assess changes in oxidative stress markers; secondary outcomes included lipid profile, insulin resistance, and clinical signs such as hirsutism and menstrual frequency. Sixty women with PCOS were randomly assigned to a Vitex group (5.8 mg daily, standardized to 0.42–0.82 mg aucubin) or placebo for 12 weeks. Compared to placebo, Vitex significantly increased total antioxidant capacity, glutathione peroxidase, reduced glutathione, total thiol, and HDL. The findings suggest a clinically meaningful benefit and support further investigation into Vitex as an adjunctive therapy.

Evidence strength: Preliminary — based on single or few small RCTs. The biological plausibility is supported by VAC's dopaminergic (LH-lowering) and phytoestrogenic activity.

5.7 Female Infertility and Luteal Phase Defects

Clinical findings in relevant studies have included a decrease in prolactin levels, lengthening of the luteal phase of the menstrual cycle, normalization of the menstrual cycle, reduction of pain, and an increase in progesterone levels. There isn't enough reliable evidence to know if chasteberry would help with sexual dysfunction or infertility (NCCIH). The most frequently referenced clinical trial in this area is Milewicz et al. (1993), a randomized, placebo-controlled double-blind study of VAC extract in luteal phase defects due to latent hyperprolactinaemia.

5.8 Anticancer Activity

Casticin has been reported to have anticancer and anti-inflammatory properties via different molecular mechanisms. In recent studies, Vitex agnus-castus seed extracts showed significant cytotoxic, DNA damage, and apoptotic effects in MCF-7 breast cancer cells. Vitex agnus-castus extract inhibited cell proliferation and induced apoptosis in human prostate epithelial cell lines. Silver nanoparticles synthesized with the extract showed potential anti-proliferative effects on HeLa cancer cells. Research into its anticancer effects is still in its early stages but holds promise.

Evidence strength: Exclusively in vitro and animal/preclinical at present. No completed human clinical trials on anticancer effects have been published to date.


6. Body Systems and Health Areas Associated with Vitex Agnus-Castus

  • Female reproductive endocrine system: The main described target of Vitex agnus-castus is female reproduction. It can up-regulate ovarian cycle and fecundity via a wide spectrum of mediators from hypothalamic, pituitary, and ovarian hormones up to intracellular regulators of proliferation and apoptosis.
  • Hypothalamic-pituitary axis: Modulation of dopamine D2 receptors on pituitary lactotrophic cells is the primary established mechanism, with downstream effects on prolactin, LH, and indirectly progesterone.
  • Breast tissue: Demonstrated benefit in cyclic mastalgia trials, attributed to prolactin-lowering effects.
  • Neuroendocrine / mood systems: Agnus castus is regarded as a nervine owing to its relaxing and calming qualities. It is often included in preparations for people suffering from premenstrual and perimenopausal anxiety and low mood.
  • Antioxidant and metabolic: V. agnus-castus exhibits a wide range of pharmacological properties, including antioxidant, anti-inflammatory, antimicrobial, and cytotoxic effects.

7. Dosage Forms and Dosages Reported in Studies

The German health authorities approved the use of V. agnus-castus for menstrual cycle irregularities, PMS, mastalgia, and menopausal disorders. The German Commission E recommended 30–40 mg/day of extract of dried fruit standardized based on 0.6% casticin.

The EU herbal monograph (EMA/HMPC) specifies a daily dose of once-daily 20 mg dry extract for the well-established use indication (PMS) in female adults.

The following dosages have been used in specific cited clinical studies:

  • 40 mg/day of chaste tree fruit extract (Agnucaston®, Biomeks, Germany) was used in a clinical study on dopaminergic properties in mastalgia patients.
  • 30 mg Vitex extract per dosage envelope was used for 8 weeks in a randomized double-blind menopausal symptoms trial.
  • 5.8 mg/day standardized to 0.42–0.82 mg aucubin was used for 12 weeks in a PCOS RCT.
  • The most extensively studied proprietary extracts in PMS clinical trials are Ze 440 and BNO 1095; these extracts were compared to placebo in the three studies meeting strict inclusion criteria in the Csupor 2019 meta-analysis.
  • Both Mastodynon® and Cyclodynon® products are approved for treatment of menstrual cycle disorders, PMS, and mastodynia, and are available as tablets and oral drops.

In all clinical trials reviewed, VAC was administered once daily, typically in the morning. Treatment duration in clinical trials has ranged from one to three menstrual cycles (approximately 3 months) for PMS/mastalgia, and up to 8–12 weeks for other indications.


8. Safety Profile, Adverse Effects, and Drug Interactions

General Tolerability

The most frequent adverse events associated with the use of A. castus are nausea, headache, gastrointestinal disturbances, menstrual disorders, acne, pruritus, and erythematous rash. Data available from clinical trials, post-marketing surveillance studies, surveys, spontaneous reporting schemes, manufacturers, and herbalist organizations have been reviewed. The adverse events following A. castus treatment are mild and reversible, indicating that A. castus is a safe herbal medicine.

Per the European Medicines Agency Community herbal monograph on Vitex, very rare side effects may include severe allergic reactions with face swelling, dyspnoea, and swallowing difficulties. No case of overdose has been reported in the EMA monograph documentation.

Pregnancy and Lactation

Research has determined that chasteberry supplementation may not be safe for women with hormone-sensitive conditions, such as breast, uterine, or ovarian cancer. Some preclinical evidence states that the use of chasteberry during pregnancy or while breastfeeding may be unsafe. In pregnancy, there is poor evidence based on theoretical and expert opinion and in vitro studies that chastetree may have estrogenic and progesteronic activity, uterine stimulant activity, emmenagogue activity, and may prevent miscarriages. In lactation, theoretical and expert opinion conflict as to whether chastetree increases or decreases lactation.

Drug Interactions

There are limited studies into the interactions between Vitex agnus-castus and oral contraceptives. However, the potential for such an interaction is recognized internationally. Health Canada and the European Medicines Agency (EMA) advise consumers to consult a health practitioner prior to using Vitex agnus-castus–containing products if they are taking hormone-containing medications such as progesterone preparations, oral contraceptives, or hormone replacement therapy. Consumers taking an oral contraceptive to prevent pregnancy should be aware that Vitex agnus-castus may interfere with how well the contraceptive works.

Consumers taking medicines with contraceptive, hormonal, and/or dopaminergic actions should consult their health professional prior to combined use with Vitex agnus-castus. Some studies indicate Vitex agnus-castus may bind estrogen and dopamine receptors, which suggests the potential for interaction with medications that have similar receptor targets.

Use in Adolescents

The EU herbal monograph specifies that there is no relevant indication in prepubertal children, and the use in pubertal children and adolescents under 18 years of age is not recommended.
