Boswellic acid, a group of pentacyclic triterpenic acids found in the resin of Boswellia species (frankincense), has a history of use in traditional medicine, especially for inflammatory conditions. Scientific research has begun to elucidate its mechanisms and efficacy, particularly regarding anti-inflammatory and analgesic effects. Several preclinical studies (in vitro and animal models) indicate that boswellic acids inhibit 5-lipoxygenase, an enzyme involved in leukotriene synthesis, thus reducing inflammation. This anti-inflammatory property forms the basis for its use in supporting the management of injuries such as sprains, strains, and soft tissue trauma, where inflammation is a key component.

However, while there are a few small human studies and case series suggesting potential benefits for musculoskeletal pain and swelling, robust clinical trials specifically targeting acute injuries are lacking. Most human research focuses on chronic inflammatory diseases like osteoarthritis or rheumatoid arthritis. Therefore, while the biological plausibility and preclinical evidence are promising, direct scientific validation for the use of boswellic acids in treating acute injuries remains limited. The current evidence base, though supportive, is not strong enough to warrant a high confidence rating. More well-designed clinical trials are needed to confirm efficacy and safety in the context of injury recovery.
